Job Title: RN Care Manager – Managed Long-Term Care (MLTC) | Hybrid | 

Location: New York, NY (Hybrid)
Schedule: Monday–Friday | 9:00 AM–5:00 PM
Contract: 8 Weeks
Openings: 2

Duties:

✅ Develop and maintain individualized care plans
✅ Coordinate care with physicians, providers, and caregivers
✅ Monitor member health status and service utilization
✅ Support discharge planning and transitions of care
✅ Conduct telephonic and in-person care management activities
✅ Identify and address barriers to quality healthcare
✅ Reduce avoidable hospitalizations and emergency visits
✅ Participate in interdisciplinary care team meetings

Qualifications: ✔ Active New York RN License ✔ Bachelor's Degree required ✔ 5+ years of clinical nursing experience ✔ Managed Care, Case Management, Care Coordination, or Utilization Review experience
