Ourtherapy team is passionate about creating personalized care plans that addressthe unique needs of each individual. Key Responsibilities Conduct patient evaluations and establish treatment goals. Implement therapy interventions to improve ADLs, fine motor skills,and functional mobility. Recommend adaptive equipment and environmental modifications asneeded. Collaborate with nursing, PT, SLP, and families to maximize patientoutcomes. Educate patients and caregivers on strategies for success at homeand in the community. Complete required documentation in a timely, accurate manner. Qualifications Current state license as an Occupational Therapist. Graduate of an accredited OT program with Bachelor’s, Master’s, orDoctorate Degree Strong organizational and problem-solving skills. Experience in Skilled Nursing/Long-Term Care preferred.
