Evening LPN/RN - Registered Nurse (LPN/RN)

About the Role:
As an Evening LPN/RN at Astoria Skilled Nursing and Rehabilitation, you will provide high-quality, patient-centered care to our residents while collaborating with our interdisciplinary team. We are seeking a dedicated and compassionate LPN/RN to join our team and make a meaningful difference in the lives of our guests.
Responsibilities:
Provide direct patient care to our residents, including administering medications, wound care, and vital sign checks
Assist with promoting independence, dignity, and well-being of our residents
Collaborate with the healthcare team to develop and implement individualized care plans
Maintain accurate and up-to-date records and reports
Participate in quality improvement initiatives and professional development opportunities

Requirements:
Current LPN/RN licensure in the state of Ohio
