Hospice Registered Nurse Case Manager (RN)

As a Hospice RN Case Manager, you will: Provide skilled nursing assessment and care in patients’ homes and communities Develop and implement individualized plans of care Assess patient and family needs and coordinate appropriate services Collaborate with the interdisciplinary group (IDG) Communicate effectively with patients, families, physicians, and team members Monitor medications, supplies, and equipment for appropriate utilization Document care accurately and maintain compliance with hospice regulations and standards Assist with On call requirements Job Requires: Current RN license in the State of Ohio Hospice, home health, or community nursing experience preferred Compassionate, organized, proactive, flexible, and dependable Strong clinical judgment and communication skills Ability to work independently while being a collaborative team member A genuine desire to provide exceptional end-of-life care Experience: NURSING : 1 year (Required) License/Certification: CPR Certification (Required) RN License (Required) Ability to Commute: Beachwood, OH and surrounding communities willingness to travel: Work Location: King David Campus Surrounding communities (limited off campus). #J-18808-Ljbffr Outcome HC
