Licensed Practical Nurse (LPN)

WCS Healthcare is seeking a Bilingual Spanish Licensed Practical Nurse (LPN) for an outpatient Mental Health Clinic in Yonkers, NY.

This is an excellent opportunity for an LPN with behavioral health, community health, outpatient, or primary care experience who is passionate about supporting individuals living with mental health conditions. The LPN will play a key role in coordinating patient care, medication management, health screenings, and supporting a multidisciplinary treatment team.

Job Responsibilities

• Administer medications and monitor patients for therapeutic response and side effects.
• Obtain and document vital signs, health assessments, and patient histories.
• Assist psychiatrists, nurse practitioners, and clinicians with patient care activities.
• Coordinate medication refills and communicate with pharmacies as needed.
• Educate patients and families on medications, treatment plans, and wellness practices.
• Maintain accurate and timely documentation within the electronic medical record (EMR).
• Monitor patient safety and report significant changes in condition to providers.
• Assist with care coordination and referrals to community resources.
• Support clinic operations and ensure compliance with regulatory requirements.
• Foster a safe, supportive, and therapeutic environment for patients.

Requirements

• Active New York State Licensed Practical Nurse (LPN) License.
• Minimum of 1 year of nursing experience preferred.
• Mental health, behavioral health, outpatient clinic, community health, primary care, or psychiatry experience strongly preferred.
• Strong communication and interpersonal skills.
• Ability to work collaboratively within a multidisciplinary healthcare team.
• Experience with EMR systems preferred.
• Current BLS Certification preferred.
