Home Health Case Manager – Northland Territory Location: Lee's Summit, 400 NW Murray Rd, Lee's Summit, MO 64081, USA JOB SUMMARY The Registered Nurse Care Team Leader (Case Manager) provides professional nursing services in accordance with physicians’ orders or plan of care to patients 18+ in the Kansas City Metropolitan Area. The RN is responsible for determining nursing needs for an assigned group of patients, initiating the plan, and evaluating its effectiveness. ESSENTIAL JOB FUNCTIONS Assess patients, process physicians’ orders, and administer prescribed medications. Coordinate nursing care and educate patients, residents and families about care. Delegate nursing tasks to staff, monitor and ensure the delivery of care. Accurately document patient progress and observations. Report changes, needs and requirements to appropriate staff. Adhere to all safety regulations and requirements. Handle stress well and maintain good workplace conduct. Provide direction to caregivers on a daily basis. Drive significant mileage in a personal vehicle as required. ESSENTIAL QUALIFICATIONS Associate’s Degree in Nursing (ADN) or higher. Registered Nurse licensure in Missouri and Kansas. Current Basic Life Support (BLS) certification or ability to obtain prior to direct patient contact. Valid driver’s license and proof of auto insurance. Basic proficiency with Microsoft Office (Word, Excel, Outlook). Strong time‑management and multitasking skills. Ability to work autonomously in a home‑health or village house setting. Effective communication and presentation skills. Solid mathematical skills (e.g., basic arithmetic, percentages). Analytical problem‑solving ability. SPECIAL WORKING CONDITIONS Exposure to infectious diseases. Work may require driving between locations and possible on‑call responsibilities. Assistance may be required in declared emergency situations. PHYSICAL AND MENTAL DEMANDS Regularly move 10–50 lbs, bend, kneel, crouch, reach, stand, walk and lift using upper body. Perform repetitive hand and wrist motions such as typing and picking. Maintain clear visual and hearing acuity for patient care and administrative tasks. Sustained periods of standing and walking between patient visits. SUPERVISION RECEIVED Daily supervision from Clinical Manager. SUPERVISION GIVEN Provides direction to caregivers on a daily basis.
