Licensed Practical Nurse / LPN - HospiceThe Licensed Practical Nurse assists in planning, coordination and care of hospice patients in accordance with the patient's plan of care. The position provides direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the interdisciplinary team to help achieve positive care outcomes. Schedule is M-F full-time 40 hours with some additional after hours/weekend back up shifts. Coverage area is Tigard, Tualatin, King City, and Wilsonville areas.Estimated Hiring Range: $33.98 - $41.53Essential ResponsibilitiesProvide direct patient care as defined in State Nurse Practice Act following a patient assessment and an approved plan of care initiated by the registered nurse. This position may be responsible for one or more of the following areas:Focused/limited nursing visitsTelephonic triageNight/weekend coverage back up rotation as shared among the nursing team in case the primary nurse is offOther duties assigned by the Hospice LeadershipCollaborate with hospice team members regarding patient care.Perform focused/limited nursing assessment, recognize signs and symptoms of end-stage medical conditions/illnesses and implements planned nursing and medical interventions as delegated by registered nurse/clinician.Perform duties in compliance with Medicare Condition of Participation.Responsible for gathering and reviewing documentation in the electronic medical records, assuring accuracy, completeness and compliance with regulations, and certification standards.Recognize signs and symptoms of common and routine medical conditions/illnesses, abnormal versus new/existing and implements planned nursing and medical interventions as delegated by registered nurse/clinician.Administer medications and treatments as prescribed by patient's prescribing clinician.
