Job Title: Licensed Practical Nurse (LPN) – Long Term Care

Location: Grand Junction, CO
Shift: Variable Nights
Schedule: 36 Hours/Week (12-hour shifts, 3 days/week)

A dedicated and compassionate Licensed Practical Nurse (LPN) is needed to support a skilled nursing and long-term care environment. This role involves delivering high-quality patient care, administering medications, monitoring resident health, and assisting with individualized care plans.

The ideal candidate is patient-focused, dependable, and able to work effectively in a team-oriented setting.

• Provide direct patient care in a long-term care setting
• Administer medications and treatments as prescribed
• Monitor and document residents’ health status and vital signs
• Assist in developing and implementing individualized care plans
• Collaborate with RNs, physicians, and healthcare staff
• Ensure accurate and timely clinical documentation
• Support residents’ comfort, safety, and overall well-being

• Active Colorado LPN License
• Ability to work night shifts and flexible schedules
• Strong communication and patient care skills

• Experience in Long-Term Care (LTC) or Skilled Nursing facilities
