MSW Social Worker

We seek a highly skilled and experienced Registered Nurse with specialties in Medical/Surgical, ICU, ER, Psychiatric, Labor & Delivery, PACU, or Operating Room to provide skilled nursing care, case management, and hospice care. The role involves patient assessments, medication administration, treatment planning, and interdisciplinary collaboration, with a focus on discharge planning. The candidate must have at least 2 years of acute hospital case management experience and be able to work weekends and holidays as required. Responsibilities:

• Provide skilled nursing care including patient assessments and medication administration.
• Develop and implement treatment plans in collaboration with the interdisciplinary team.
• Manage case load with a ratio of approximately 1:35 patients.
• Focus on discharge planning to ensure smooth patient transitions.
• Cover up to 2 units on weekends as scheduled.
• Participate in holiday coverage as required.

• Minimum 2 years of acute hospital case management experience.
• Active Registered Nurse license in the state of practice.
• Clinical assessment, medication administration, and treatment planning skills.
• Interdisciplinary team collaboration skills.
• Excellent communication and interpersonal skills.
• Bachelor's degree in Nursing or related field.
• Driver's license, car insurance, and automobile for field work (if applicable).

• Experience in Medical/Surgical, ICU, ER, Psychiatric, Labor & Delivery, PACU, or Operating Room settings.
• Experience with discharge planning and case management in acute care.

• Weekend coverage required every other weekend.
• Must be available for holiday coverage.
• Return to Office (RTO) restrictions limit time in office to no more than 7 days.
• Work environment is an acute care facility.
• Scrub or business casual attire required.

• Work schedule includes weekend shifts every other weekend.
• Shift assignments may include coverage of up to 2 units on weekends.
• Holiday shifts as assigned.
