What You’ll Do

As a Home Health LPN, you will provide compassionate, hands-on care while collaborating closely with the Registered Nurse and interdisciplinary care team.

Your responsibilities will include:
  • Providing direct patient care in accordance with the state Nurse Practice Act

  • Carrying out individualized plans of care developed by the Registered Nurse

  • Educating patients and families on treatments, medications, and self-care

  • Completing accurate and timely clinical documentation

  • Coordinating with the RN and care team to ensure comprehensive patient support

  • Following proper procedures for equipment, supplies, and infection control

  • Supporting patients’ physical, emotional, and educational needs in the home

  • Communicating changes in a patient’s condition to the appropriate clinical team members


What You’ll Need

  • Active LPN or LVN license in the state of Tennessee

  • Graduate of an accredited practical or vocational nursing program

  • Three or more years of nursing experience preferred

  • Home Health, Medical-Surgical, or Community Health experience preferred

  • Current CPR certification

  • Reliable transportation

  • Valid driver’s license and current automobile insurance
