Employment Type: Full time Shift: Rotating Shift Description: TheNurseNavigatorisakeymemberofthemultidisciplinaryteamfunctioningasanadvocate,educatorandfacilitatorofcare. Responsibleforensuringallreferredpatientsreceivequalityandcomprehensiveservicesthroughoutthecontinuum. Coordinatestheoverallsubspecialtyprogrammaticgoalsandoutcomes. Minimum qualifications Bachelor'sdegree in Nursing CurrentStateofMichiganRegisteredNursingLicense.CPRorBCLSCertified. Three years of experience in an acute care setting. One year of experience in medical, surgical or radiationoncology(foroncologyspecialty). What the Nurse Navigator will do: Completes a comprehensive health assessment of the patient's physical, psychological, social, environmental,financialandfunctionalstatusincollaborationwithmultidisciplinaryteammembers.Includesassessmentofpastandpresenttreatmentcourse. Assistsinorganizingandcoordinatingtimelypatientappointments. Understands and develops goals, treatmentplanand provider options in concordance withevidence basedstandards and clinicaltrials. Worksdirectlywithphysicianleadtoreviewdiagnosticstudies,clinicalhistoryandcourseoftreatmenttoidentifymostappropriatenextcourse ofcare. Collaborateswithmultidisciplinaryteammembersinperformanceimprovementactivitiesincludingassessment,planning,implementationandevaluationofclinicaloutcomesand processesfor patients. Usescritical thinking andproblem solvingskills to analyze and synthesize data and formulatean appropriate casemanagementplantoaddressthe patient'sindividual andfamilyneeds.
