At our organization, providing care is not merely a task; it defines who we are.

If you are a nurse eager to utilize your complete clinical expertise, engage in critical thinking, and significantly influence patient outcomes, working in Home Health presents a unique and often more fulfilling approach to nursing.

You will have the chance to make decisions, navigate complexities, and assist patients in recovering and thriving within their own homes.

Why Choose Home Health with Us
  • Clinical Autonomy & Ownership: You will take charge of care rather than waiting for directives. You will assess, make decisions, and adapt in real-time, taking responsibility for your patients' outcomes.
  • Complex, High-Impact Care: You will provide care for patients with a variety of complex needs, from post-acute recovery to chronic condition management, requiring strong clinical judgment and flexibility.
  • Real Work-Life Balance: Enjoy flexible scheduling and generous paid time off, allowing you to cultivate a sustainable career while maintaining your impact and opportunities for growth.
  • Stronger Patient Connections: You will foster meaningful relationships with patients and their families, witnessing the direct effects of your care over time.
  • A Supportive Team: Independence does not equate to isolation. You will receive robust support from respected leaders in the field, benefit from interdisciplinary collaboration, and have clear processes in place.
Role Overview

As a Home Health RN, you will serve as the clinical leader for your patients, overseeing the care plan, coordinating with various disciplines, and driving outcomes from admission to discharge.

You will integrate advanced assessment skills, care planning, and patient education to minimize hospitalizations, enhance recovery, and promote long-term independence at home.

Your Responsibilities
  • Conduct thorough in-home assessments to evaluate clinical status, risks, and care objectives.
  • Develop and manage personalized care plans in collaboration with physicians and care teams.
  • Provide skilled nursing care, including medication management, treatments, and interventions.
  • Act as the case manager, coordinating care among therapists, aides, and physicians.
  • Monitor patient progress and make clinical decisions based on evolving conditions.
  • Educate patients and families to facilitate disease management and independence.
  • Coordinate care transitions and discharge planning to minimize readmissions.
  • Maintain timely and accurate documentation in the electronic medical record (EMR).
  • Supervise and support Home Health Aides as necessary.
  • Participate in on-call rotation as required.
What You Bring
  • Active RN license (or eligibility for compact license) and CPR certification.
  • Graduation from an accredited nursing program.
  • At least one (1) year of RN experience (preferably in Home Health, Med-Surg, ICU, or Acute Care).
  • Strong clinical judgment and the ability to think independently.
  • Excellent communication skills with a patient-centered focus.
  • Capability to manage a caseload and prioritize effectively.
  • Proficiency in using EMR systems.
  • Valid driver's license and dependable transportation.
What Sets You Apart
  • Experience in home health or post-acute care settings.
  • Background in case management and interdisciplinary coordination.
  • Bachelor of Science in Nursing (BSN).
  • Familiarity with Homecare Homebase (HCHB).
What to Expect
  • A field-based role providing care to patients in their homes within a designated area.
  • A high level of autonomy supported by a collaborative clinical team.
  • A dynamic work environment that requires adaptability, critical thinking, and effective time management.
  • Performance expectations linked to clinical quality, patient outcomes, and timely documentation.
