Responsibilities

• Provide Physical Therapy Assistant services under supervision, executing treatment plans for patients in the LTAC/long-term care setting.
• Assist with therapeutic exercises, gait training, transfers, range of motion, and activities of daily living as directed by the supervising PT.
• Monitor patient responses to therapy, document progress, and communicate changes to the PT and care team; maintain accurate, timely records.
• Maintain infection control, safety during mobility and transfers, and adherence to standard precautions.
• Collaborate with an interdisciplinary team to adjust interventions and educate patients and families on home programs and safety strategies.

Requirements

• Active Physical Therapy Assistant license in Connecticut (or eligible to obtain).
• Experience in long-term care, LTAC, or related rehabilitation settings preferred.
• Ability to work in a LTC setting with supervision during an 8.5-hour shift.
• Strong communication and documentation skills; reliable team member.
• CPR/BLS certification or ability to obtain prior to start date.
