Key Responsibilities

• Screen, evaluate, and re-evaluate patients to assess functional ability and determine appropriate treatment plans.
• Provide direct occupational therapy treatments and select appropriate therapeutic modalities.
• Document evaluations, treatment goals, and objectives in accordance with care standards.
• Communicate and coordinate treatment activities with interdisciplinary care team members.
• Educate patients and family members on the rationale for treatments and procedures.

Required Qualifications

• Active Occupational Therapist license in Vermont and New Hampshire, or an unencumbered compact license.
• Bachelor of Science in Occupational Therapy or completion of a postgraduate Occupational Therapy Certification program.
• Must have passed the National Registration Exam for Occupational Therapy.

Preferred Experience / Skills

• Prior experience in a home health setting.
