Your New Role:


Position: Family Nurse Practitioner (FNP)


Job Duration: Full Time/Permanent


Setting: Outpatient clinic


Schedule: Monday through Friday, no call


Experience:


At least 3 years of primary care family medicine required


Must have chronic disease management in clinic experience


If a newer FNP graduate, must have ED or ICU nursing experience


Patient Population: All ages, underserved patient population in an impoverished Medford neighborhood

As a Family Nurse Practitioner in Jackson County, OR, you will be part of a team providing comprehensive medical care to a large pediatric and adult population. You will be responsible for managing chronic diseases and delivering primary care services to patients of all ages. The health center serves as a primary care medical home for many of its 28,000 patients, emphasizing accessible and exceptional health care. You will work in a neighborhood health center where many patients live nearby and walk to their medical home.
