- Licensure: Nursys, License/Certificate Verification required for submission but not for onboarding. - Onboarding Requirements: Ascension Pharmacology Exam, Ascension Specialty Exam, Ascension Core Competencies, and Ascension Unit Specific Checklist are required for onboarding. - Health Requirements: Ascension Color Vision, MMR, Fit Test, Hepatitis B, Health Statement, 2 Step PPD OR IGRA (Quantiferon/Tspot), Varicella, Tdap needed for onboarding. - Professional Documentation: Ascension Education Attestation, Ascension Nursing Contract Worker Onboarding, Ascension Badge Photo, Driver's License/State ID for submission, Ascension Profile, Ascension 2 Professional References for submission, and more requirements for onboarding. - Additional Testing: Positive PPD-Negative Chest X-Ray and Annual TB Questionnaire, Influenza Vaccine optional for onboarding. - Background and Verification: Ascension 12 Panel Drug Screen, Ascension Criminal Background Check Results, Ascension EPLS for onboarding. - References and Evaluations: Ascension Employment Verification, Ascension Education Verification, Ascension Confirmation of Assignment. - Optional: Hiring Manager Evaluation, Ascension Name Change Document.
