Medical Social Worker

PRN Licensed Master Social Worker

We're hiring a PRN Licensed Master Social Worker to provide in-home medical social work across the Fredericksburg region. You'll be meeting patients where they are — assessing their social, emotional, and environmental needs, supporting families through difficult transitions, and connecting them to the resources that help them live safely at home. This role is a good fit for LMSWs who want clinical autonomy and real relationship-driven work without committing to full-time hours.

What You'll Do:

• Pick up visits based on your availability — you tell us when you're free, we route patients your way
• Conduct social work assessments in patients' homes
• Identify social, emotional, and environmental needs
• Provide counseling and emotional support to patients and families
• Assist with community resources, discharge planning, and care coordination
• Connect patients to housing, financial, and support services
• Coordinate with our clinical team — nurses, therapists, and physicians — on care planning
• Document in the field on a company-issued tablet through WellSky

What We're Looking For:

• Active LMSW or LCSW license in D.C., Maryland, or Virginia (or eligible)
• 1+ year of clinical or medical social work experience
• Home health or healthcare experience preferred
• Strong knowledge of community resources and care coordination
• WellSky experience is a plus but not required
• Valid driver's license and reliable transportation

Service Areas:

Based out of our Fredericksburg office, we cover Fredericksburg and surrounding communities — Stafford, Spotsylvania, King George, Woodbridge, Dumfries, Culpeper, Warrenton, and Orange. You'll generally work the area closest to where you live.
