Travel LCSW - Full Time Contract

Licensed Clinical Social Worker Opportunity

Make a meaningful impact as a Licensed Clinical Social Worker with Jackson Therapy Partners. We're seeking an experienced LCSW who will advocate for patients to help them gain access to available resources while confronting personal challenges such as serious illness, grief, and psychosocial needs. Minimum Qualifications

• Master's degree in social work (MSW) from an accredited educational program
• Current LCSW or equivalent state clinical social work license in good standing with Washington State licensing board
• HCHB (Homecare HomeBase) experience required
• Experience working in hospice, palliative care, or home health settings preferred
• Caseload management experience supporting 50–60 active patient cases is preferred
• Two years of supervised clinical experience preferred

Assignment Details

• Facility Type: Home Health (Hospice specialty)
• Shift Name: Days
• Shift Details: Days - 8 hour shifts, Monday through Friday
• Typical Schedule: M-F 8:00 AM–5:00 PM (5x8)
• Guaranteed hours per week: 40
• Territory: Lewis County or potentially Thurston County; travel within the territory may be required
• Current caseload expectations: approximately 50–60 active cases
• HCHB electronic medical record experience required
