Your role includes providing clinical leadership for Managed Care, Bundle Payment, and ACO programs, acting as an educator and advocate for both patients and staff. You'll collaborate across departments to evaluate and direct activities that support patient care delivery, oversee data collection for quality indicators, and identify successes and areas for improvement. By developing comprehensive care plans with input from patients, families, and healthcare teams, you ensure smooth transitions from inpatient care to the next level.Additionally, proactive post-discharge follow-ups and consultations with physicians will empower you to eliminate barriers for patients, while your innovative ideas will continually enhance healthcare delivery systems and results. A valid Degree in Nursing as a Registered Nurse is a must, along with at least two years of experience in a hospital, nursing care facility, or similar healthcare setting. Comprehensive knowledge of managed care rules, regulations, and guidelines is essential, empowering you to navigate the complexities of health insurance and case management with confidence. You'll exhibit strong negotiation skills to ensure coverage and timely completion of case management reports. Experience with a Health Plan or third-party case management company will give you an edge in this role.Most importantly, we seek qualified, compassionate professionals dedicated to making a difference and delivering exceptional patient care.
