Mekks Home Health Care LLC is seeking a compassionate and skilled Physical Therapist (PT) to join our growing home health care team. The ideal candidate will provide high-quality, patient-centered care to individuals in their homes, helping them regain mobility, improve physical function, and enhance their overall quality of life.

Key Responsibilities

Conduct comprehensive physical therapy evaluations in patients’ homes

Develop individualized treatment plans based on patient needs and goals

Implement therapeutic exercises, manual therapy, and mobility training

Monitor patient progress and adjust treatment plans as needed

Educate patients and family members on exercises, safety, and injury prevention

Maintain accurate and timely clinical documentation in compliance with state and federal regulations

Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care

Ensure compliance with all home health care standards and company policies

Qualifications & Requirements

Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program

Active Massachusetts Physical Therapist License

Minimum of 1 year experience (home health experience preferred but not required)

Strong clinical assessment and patient care skills

Excellent communication and interpersonal skills

Ability to work independently and manage time effectively

Reliable transportation and valid driver’s license

Preferred Skills

Experience with geriatric patients and chronic conditions

Familiarity with electronic medical records (EMR) systems

Strong organizational and documentation skills

Work Schedule
