We foster a supportive culture where every team member is empowered to grow, contribute, and make a meaningful impact on the lives of patients, families, and the communities we serve.What We Offer:Company vehicle and fuel cardCompetitive salaryComprehensive benefits package, including:Medical, dental, and vision insuranceLife insuranceAD&D insuranceShort- and long-term disability coverageEmployee Assistance Program (EAP)401(k) with a 4% company matchPaid time off and paid holidaysOngoing professional development and career advancement opportunitiesA collaborative, mission-driven team committed to delivering exceptional patient careLocation: San Jose Northeast and SoutheastSchedule: Monday - Friday with on call weekends rotationAs a Home Health Aide, you will:Provide compassionate personal care and assistance with activities of daily living, including bathing, grooming, dressing, oral hygiene, toileting, skin care, and maintaining a clean and orderly environment.Prepare and assist with meals and feeding as needed, while supporting patients' nutritional and hydration needs.Assist patients with ambulation, exercise, range-of-motion activities, and other care activities as directed by the Plan of Care and within the HHA's training and scope.Monitor and record vital signs and other patient observations as ordered and within established training requirements.Assist patients with medication self-administration and other routine care needs as directed by the supervising nurse or therapist.Observe patients for changes in condition and promptly report concerns or significant changes to the supervising nurse and case manager.Maintain a safe, respectful, and supportive care environment while completing accurate documentation and following LORIAN's care standards, procedures, and professional expectations.Required QualificationsMust meet educational/experience requirements outlined in the laws of licensure states.Certified/licensed and in good standing within state currently working in according to clinical type.Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.Possesses and maintains current CPR certification.Possess and maintains a valid driver's license.This position is based on a pay per visit compensation model.
