(RN) Palliative Nurse Case Manager - Per Diem

This Per Diem Nurse Care Manager (NCM) will support the outpatient Palliative Care team by providing clinical care coordination, patient triage, symptom management support, and care transitions for medically complex patients. Responsibilities include managing patient communications via phone or MyChart, triaging urgent clinical needs, coordinating care across inpatient, outpatient, and community settings, supporting symptom management and medication reconciliation, facilitating referrals (hospice, home health, specialty services), and partnering closely with physicians and APPs to ensure continuity and quality of care. Clinical knowledge of symptom and pain management and experience working with multidisciplinary teams to optimize patient care are required. Strong clinical, communication, and advocacy skills are essential. Autonomous nursing care requires the ability to assess and provide appropriate nursing actions based on competence, professional expertise, and knowledge. Provided care is consistent with ANA Standards of Practice, and independent judgment is exercised within interdisciplinary contexts. Cover Letter: While not required, a cover letter is highly recommended when applying to this position. Minimum Qualifications Graduate from an accredited nursing school. Registered Nurse (RN) license issued by the State of California. BART or BLS skills at time of hire with commitment to obtain BART within 6 months. At least one year of outpatient RN experience caring for patients at the end of life or with life‑limiting progressive illnesses. Recent experience processing prior authorizations. Proven ability to collaborate effectively with interdisciplinary teams to manage a caseload, including intake and triage of symptom calls and coordination of care. Excellent verbal and written communication skills, conveying complex medical information clearly and compassionately to patients and families. Ability to provide individualized care to vulnerable populations. Preferred Qualifications BSN or MSN degree. Experience with hospice or palliative care. Bilingual (English/Spanish). Special Conditions Must be able to work various hours and locations based on business needs. Employment subject to a criminal background check and pre‑employment physical. Pay Transparency Act Annual Full Pay Range: $190,342 – $190,342 (prorated if the appointment percentage is less than 100%). Hourly Equivalent: $91.16 – $91.16. Factors determining compensation include experience, skills, knowledge, abilities, education, licensure, certifications, and other business needs.
