Registered Nurse (RN) – Hospice Care | Night Shift | On-Call Only Location: Colorado Springs / Woodland Park, CO Area Schedule : Monday to Friday Night Shift: 5:00 PM to 8:00 AM (Shift ends Friday morning) On-Call Only Guaranteed 40 Hours Per Week Mileage Reimbursement: $0.45 per mile Required Skills & Experience: Hospice Care experience required HCHB (Homecare Homebase) experience required License Requirements: Active Colorado RN License or Multi-State RN License Reliable Transportation Required Valid Car Insurance Required Job Summary: We are seeking an experienced Registered Nurse (RN) with Hospice and HCHB experience to provide high-quality patient care in the Colorado Springs and Woodland Park area. The ideal candidate will be responsible for conducting assessments, coordinating patient care, supporting treatment compliance, and collaborating with healthcare professionals to ensure the best patient outcomes. Primary Responsibilities: Perform PRN/focused assessments for high-risk patients Develop and implement individualized patient care plans Coordinate care transitions and collaborate with physicians, social workers, and healthcare teams Identify appropriate and cost-effective levels of care Assist patients with non-medical support needs such as transportation and housing resources Maintain accurate clinical and patient outcome documentation Promote health maintenance and illness prevention Act as a patient advocate while maintaining confidentiality and compliance standards Engage specialty resources as needed to support patient care Qualifications: Associate’s or Bachelor’s Degree in Nursing or related field Current unrestricted RN License Strong clinical assessment and critical thinking skills Ability to analyze complex medical information and make sound decisions Case Management certification preferred Employment Type: Full Time Salary: $55 - $60 Hourly Bonus/Commission: No
