Key Responsibilities\n

  • Provide empathetic counseling and emotional support to patients and their families, helping them navigate adjustment challenges and enhance their emotional well-being.

  • Work closely with the rehabilitation team to develop and implement individualized treatment plans that address each patient’s unique psychosocial needs, integrating insights from various healthcare professionals.

  • Guide patients in accessing essential community resources, including support groups, vocational rehabilitation, and housing assistance to support their recovery.

  • Educate and empower patients and their families about available community resources and post-rehabilitation services, ensuring a smooth transition back into daily life.

  • Take an active role in discharge planning, facilitating smooth transitions for patients moving to home or other care settings.

Patient Population / Client Focus\n

As a Licensed Clinical Social Worker at BayCare, you will work primarily with patients undergoing rehabilitation for various conditions, including neurological disorders, orthopedic surgeries, stroke recovery, and spinal cord injuries. Our patient population is diverse, encompassing individuals of different ages and backgrounds, each facing unique challenges that require tailored interventions.   • A Master’s degree in Social Work from an accredited program.

  • Active state licensure as a Licensed Clinical Social Worker (LCSW) in Tennessee.
