Title
Acute Hospital Case Management

Schedule
- Weekend Requirement: Every other weekend
- Holiday Coverage: Must be available to work 3 of 5 major holidays

Assignment Details
- Patient Ratio: 1:35
- Caseload: Up to 25 patients
- Orientation: Approximately 40 hours

Requirements
- Minimum of 2 years acute hospital case management experience
- Travel experience required
- Experience with discharge planning and care coordination
- Comfortable working with patients across all age groups (neonates to geriatrics)
- California state license preferred; pending license accepted
- Experience with mental health crises, involuntary holds, and vulnerable populations
- Familiarity with the Epic documentation system

Roles & Responsibilities
- Coordinate patient care and discharge planning across interdisciplinary teams
- Conduct biopsychosocial assessments and develop comprehensive treatment plans
- Facilitate family meetings and educate patients and caregivers on care plans
- Manage prior authorizations and determine benefits eligibility
- Document care aligned with coverage criteria

Compliance / Notes
- Return-to-Office restriction: No more than 7 days
- Dress Code: Scrubs or business casual
- References: 2 required covering at least 1 year within last 3 years
