Job Description
Title
Case Management - Acute Care

Schedule
- Every other weekend required
- Must commit to working 3 of 5 major holidays

Requirements
- Minimum 2 years recent acute hospital case management experience
- Acute care case management experience must be current within the last year
- Experience with HMOs/IPAs and utilization/benefits processes
- Proficient in Epic

Roles & Responsibilities
- Manage a caseload of up to 25 patients per day, coordinating across interdisciplinary teams to facilitate discharges.
- Conduct needs assessments and develop discharge plans that address complex patient needs.
- Oversee the authorization, denial appeals, and benefits eligibility for patients under care.
- Ensure compliance with CMS/JCAHO core measures, working with advance directives and protective services as needed.
- Familiarity with processes related to worker's compensation is beneficial.

Compliance / Notes
- Free parking provided on the first day; assignment parking available during orientation.
- Failure or refusal to float based on department needs may result in assignment termination.
