RN Home Health Case Manager – Milwaukee North (West Allis, WI)
If you're a home-health-savvy RN who's all about strong assessments, independent workflow, and real continuity of care, this contract is a solid fit. Straight weekday schedule, no call, no weekends—very classic HH setup.
Shift & Schedule
- Monday–Friday
- 8:00 a.m.–4:30 p.m.
- 40 hours/week
- No call
- No weekends
- Holidays as needed
- May float/assist other HH teams
Coverage Area (North Milwaukee Zone)
53097, 53092, 53223, 53217, 53209, 53224, 53218
Required Experience
- Minimum 1 year Home Health RN experience
- Must be confident managing:
- OASIS documentation
- Start of Care visits (target: ~3 SOCs/week)
- Patient teaching and independent care planning
- Strong computer documentation skills
- Valid WI or compact RN license
- Active BLS (AHA)
Clinical Skills Needed
- Wound care: wound vacs, pleural drains, ostomy care
- Peripheral blood draws
- Central line care
- IV administration (elastomeric devices, IV pumps)
- High-level assessment and triage judgment
- Ability to work independently in the field
Other Requirements
- Reliable transportation
- Valid driver's license
- Current auto insurance
Orientation
- Standard onboarding through the Home Health team
- Additional support available through other teams if floating occurs
