Responsibilities include assessment & intervention for barriers to care, acute safety concerns, transition-to-adulthood planning, patient and family coping/stress, and general support as families navigate the healthcare and community systems. As the SW Clinic Leader, they are an active member of the CP Leadership Team, supporting QI initiatives and program development. Additionally, the Clinical Leader provides direct supervision to other social workers within the department and contributes to SW department leadership initiatives.****Job Description Summary:**Provides team leadership for the provision of Clinical Medical Social Work services for patients and their families.
