# Housing Case ManagerZumbro Valley Health Center - Rochester, MN 55904## OverviewSalary Range$25.11 - $30.13 HourlyPosition TypeFull Time## DescriptionWe are looking for a **Full-time Housing Case Manager** to provide Housing Support and Case Management services to individuals that have a history or are currently experiencing homelessness. This position requires a compassionate, resourceful, and highly organized individual who can effectively collaborate with a team to help empower clients to achieve stability and independence.**Responsibilities:*** Coordinate, make referrals, and follow up for services and resources as needed.* Complete all admission and discharge paperwork in a timely manner.* Attend and participate in case consultations, and supervision meetings.* Assist clients with securing housing, and interface with landlords to maintain housing.* Provide support services once housed to help improve stability and longevity in housing.* Use motivational interviewing techniques and engagement strategies to build an alliance with clients.## Qualifications**Requirements & Qualifications*** Bachelor's degree in Social Work, Psychology or a related human services field.* 1 year of experience providing services to individuals with a mental illness or substance use disorder required.* Experience working with housing clients preferred.* Must qualify as a Mental Health Practitioner in the State of MN.* Licensed as a Social Worker in the State of MN preferred.* Provisional License in the State of MN will be considered.* Valid driver's license and proof of auto insurance.**Why Zumbro Valley Health Center?**Since 1966, Zumbro Valley Health Center has been serving our community as a non-profit organization.
