Nurse Practitioner, Longevity Care in Senior Living (In-Person Contract)

Nurse Practitioner - Longevity ProgramTotal Life connects adults 65+ with licensed clinicians for behavioral health, geriatric care and longevity services, covered by Medicare, Railroad Medicare and major commercial insurers. Since 2021 we've grown to more than 200 credentialed providers serving patients in 49 states.Most longevity medicine is built for healthy 40-year-olds. Ours is built for older adults. The Longevity Program combines a comprehensive geriatric assessment with chronic disease management, cognitive health, medication review and weight management, organized around five pillars of healthy aging: sleep, nutrition, movement, mental health and social connection. It works alongside a patient's existing care and never replaces their primary care provider.This is an in-person contract role. We're looking for nurse practitioners in the Inland Empire area to lead longevity days at our partner senior living communities. Each day opens with a short educational talk for residents on healthy aging, using a presentation and speaker notes we provide. You then see the residents who sign up for one-on-one longevity visits.You'll be trained and supported by our Chief Medical Officer, a board-certified geriatrician who designed the program's clinical content. Your first site day is run alongside a member of our team.Site days are on weekdays. The presentation is the fixed part of the day. The rest depends on how many residents sign up. Expect roughly 4 to 8 hours per site day, a few times a month to start, with hours growing as bookings grow.What you'll do:Conduct 45 to 60 minute longevity visits built on a comprehensive geriatric assessment: what matters most to the resident, medication review and deprescribing, cognition (including the Mini-Cog), mood, sleep, nutrition, mobility, fall risk, daily function and advance care planningEvaluate eligible residents for weight management, including GLP-1 therapy where clinically appropriateIdentify residents who would benefit from follow-up care, such as a fuller cognitive review, ongoing medical follow-up, psychiatry or therapy, and coordinate with their primary care providerDocument every encounter accurately and completely, to Medicare standards, in our EHRRepresent Total Life professionally with residents, families and community staffTravel to assigned senior living communities in the Inland Empire areaRequirements:Active, unrestricted California RN license and Nurse Practitioner certification from the California Board of Registered NursingAn active NP Furnishing Number from the California Board of Registered NursingCurrent national NP board certificationActive DEA registration for California and registration with CURES, California's prescription drug monitoring programAbility to practice under standardized procedures with a supervising physician, as required in California, or certification as a 104 NP under AB 890 (a 103 NP must practice in a setting with a physician on site, which our senior living sites do not have)Clinical experience caring for adults 65+Experience with Medicare documentation and E/M codingYour own malpractice insurance (minimum $1M per occurrence, $3M aggregate) that covers in-person care, not just telehealth, or the ability to obtain it before your first site dayComfort presenting to groups and speaking with older adults and their familiesA valid driver's license and reliable transportationAbility to complete each community's onboarding requirements, which may include a background check, TB screening and immunizationsWeekday availability for on-site days in the Inland Empire area, with hours confirmed in advanceNice to have:Experience with comprehensive geriatric assessmentFamiliarity with cognitive screening and staging tools (Mini-Cog, MoCA, FAST); MoCA certification is a plusExperience in senior living, long-term care, SNF, hospice, palliative or house-call settingsExperience prescribing GLP-1s for weight managementTelehealth experienceLicenses in more than one stateAdult-Gerontology NP certification (AGNP/AGPCNP), or Family NP with a geriatric focus or trainingWhat we offer:Mileage reimbursement for site daysTraining from our CMO in geriatric assessment, cognitive tools (Mini-Cog, MoCA, FAST) and our documentationPresentation materials, resident sign-ups and scheduling handled by our teamFlexible contractor schedule, with hours that grow as your calendar fillsThe chance to help build a program for a population the longevity field has overlookedTotal Life welcomes applicants of all backgrounds and is committed to equal opportunity.
