In this role, also known as a Continued Stay Review Assessor, you’ll conduct in‐person follow‐up visits to confirm individuals continue to receive the appropriate services and supports.You’ll play an important role in reviewing ongoing eligibility for long‐term care services, completing assessments in alignment with program guidelines, and ensuring confidentiality and accuracy every step of the way.If you’re motivated by purpose‐driven work and enjoy combining clinical insight with compliance‐focused assessments, this role offers the opportunity to make a real difference.\*This position involves traveling up to 90% within designated regions of Central Illinois.\***About the program**Maximus partners with the State of Illinois Department of Healthcare and Family Services (HFS) to manage the assessment process for Preadmission Screening and Resident Review (PASRR) Level I screens and completing Level II Serious Mental Illness (SMI) evaluations for individuals applying to, or residing in, Medicaid-certified nursing facilities.
