This is a full-time, 100% outpatient opportunity offering a collaborative, team-based environment with a strong emphasis on preventive care, chronic disease management, and improving quality outcomes for seniors. This role is ideal for a physician who is passionate about delivering high-quality, relationship-driven care and is comfortable working with Medicare Advantage patients in a model that prioritizes access, coordination, and long-term health outcomes.
