Hybrid Housing Care Manager: Homelessness & Care Coordination

MetroPlus

MetroPlus in New York City is seeking a Housing Taskforce Care Manager to provide critical support to members experiencing homelessness. This role involves optimizing healthcare delivery and advocating for members through their care journeys. The ideal candidate will have a Master's Degree in Social Work and at least three years of experience in care management. The position combines remote and field work, with a focus on collaboration and problem-solving.
