Role – Specialty Physical Therapist – Rehabilitation

Schedule Day Shift

Assignment Details Contract Length: 13 weeks Guaranteed Hours: 40

Requirements Active license as a Physical Therapist in the applicable state Basic Life Support (BLS) certification preferred Prior experience in a long-term care or skilled nursing facility preferred

Roles & Responsibilities Provide physical therapy services to maximize residents' functional abilities and independence in daily living activities Evaluate residents and establish a therapeutic plan of care with functional goals Consult with referring physicians and healthcare team members on care plans and discharge needs Document all evaluative and therapeutic interactions in the Electronic Medical Record Educate residents and caregivers on therapy goals and procedures Collaborate with other therapy professionals to ensure resident care quality

Compliance / Notes Maintenance of required licenses, certifications, and competencies is essential Adhere to all infection control procedures Participate in department orientation and staff mentorship
