Outreach Specialist

JOB OPENING Vida Y Salud Health Systems, Inc., a Federally Qualified Health Center (FQHC), is currently seeking a qualified individual who will fill a full-time Outreach Specialist position. Requirements: Collaborate with local community partners to identify and engage those individuals who might not otherwise seek assistance or come for healthcare and/or behavioral health services. Serve as a health motivator offering patients in the program encouragement and support. Influences patients in the program to make lifestyle changes. Engaging with identified individuals and ensuring that their basic needs are met while supporting them along their pathway. Perform various clerical functions by ensuring the proper completion and handling of the required forms as well as tracking program participation and attendance. Education: High School Graduate or G.E.D. equivalent. Ability to communicate in oral/writing form on information and ideas is essential to duties, maintaining good working relationship with staff and the community. Familiarity with medical terminology. Bilingual English and Spanish preferred. Must be computer literate. Must have a valid Texas Driver’s License and current automobile insurance.
