Licensed Practical Nurse (LPN) - Director of WellnessWe are seeking an experienced Licensed Practical Nurse (LPN) to join our team as Director of Wellness at Rocky Mountain Assisted Living. As a key member of our care team, you will be responsible for overseeing the direct and indirect care of our residents, ensuring medication management, outside therapies, and activities of daily living are utilized to create an optimal level of care in a home-like environment.Qualifications:Current LPN or RN licensure in Colorado, with at least one year of experienceA desire to work with and a sympathetic attitude towards the elderlyAbility to read and write in English and follow verbal and written instructionsBasic knowledge of Colorado State Health and Environment regulations pertaining to resident careExperience working in a setting that provides care to the elderlyAbility to identify resident's physical, mental, and psychosocial needsAbility to work closely and professionally with Rocky Mountain Assisted Living staff, families, residents, outside agencies, and vendorsAbility to remain patient, poised, and dignified in emergency or crisis situationsKnowledge in dementia care or ability to obtain training certification from Alzheimer's Association within 30 days of hireAbility to maintain confidentiality of employee and resident informationAbility to sit or stand for prolonged periods of timeAbility to prioritize and multitask while maintaining a high level of organizationBasic computer skills, including the use of Word, Excel, Power Point, and OutlookLine of Authority:Reports directly to the Executive DirectorResponsibilities:Communicates professionally with residents, family members, staff, outside agencies, and vendorsAssesses potential and returning residents in their current living environment, making sure the individual is assisted living appropriate based on company and accepted industry standardsAssists with move-ins and discharges, ensuring accurate and complete paperwork and communication with physicians, families, and outside agenciesCompletes resident care plans that meet the resident's mental, social, and psychosocial needsConducts in-services for staff on topics related to new move-insCoordinates resident discharges and ensures requested paperwork is delivered in a timely and professional mannerEnsures homes are compliant with Colorado State Health Department regulations related to care and medication managementMaintains accurate and complete Medication Administration Records for each residentPerforms audits to ensure accuracy and completeness, and ensures physician orders are compliant with company guidelines and regulationsMonitors resident's prescribed therapies and communicates with outside agencies to ensure quality careEvaluates residents frequently, looking for changes in resident condition, and adjusts care plans accordinglyReports changes in resident condition to the Resident Care Coordinator and Executive DirectorWorks closely with care staff, House Managers, Resident Care Coordinator, and Executive Director to assess residents' needs and determine if they continue to have a level of care appropriate for assisted livingIf you are a motivated and compassionate LPN looking for a new challenge, we encourage you to apply for this exciting opportunity.
