Job Description Summary:

\n

To competently interpret the patients history and baseline level of function and then to complete an assessment appropriate to the patients areas of deficits. Based on problems identified, the physical therapist establishes and carries out a plan of care to include patient treatment and reassessment, patient/family instruction, discharge planning, documentation in the medical record and communication with physicians and other medical staff. This position helps patients achieve maximum functional outcomes. Acts as a clinical instructor for students and actively participates in department functions.\n

Responsibilities And Duties:

\n

Direct Patient Care 90-100%\n
\nEvaluates and treats patients:\n
\nAfter provider referral, evaluates patient physical abilities and functional status, develops and implements plan of care based on evaluation findings and appropriate treatment techniques and initiates patient treatment.\n
\nCarries out plan of care with reassessment and changes to plan of care as appropriate.\n
\nResponsible for maintaining/meeting the productivity expectations of their department.\n
\nOversees and supervises provision of care by physical therapist assistants.\n
\n \n
\nPlans and helps coordinate discharge plan in collaboration with the multidisciplinary team:\n
\nInstructs patient and care giver/family.\n
\nRecommends DME required for discharge.\n
\nProvides information to providers, nurses, case managers, social workers, and other team members.\n
\nInvolved in the multidisciplinary patient plan of care and discharge readiness plan.\n
\n \n
\nDocuments evaluation, plan of care, progress toward goals, discharge plan/continuity of care for multidisciplinary team, and other involved medical and insurance parties.\n
\n
\nIndirect Patient Care 1-10%\n
\nDepartment Functions:\n
\nSupervises patient assignments and duties of the PTAs and rehabilitation tech(s).\n
\nIs involved in department quality improvement through data collection and participation in team and staff meetings.\n
\nAssumes duties delegated by management in the absence of department management.\n
\n
\nEducation/Mentoring:\n
\nTakes an active role in orienting new staff members.\n
\nServes as clinical instructor for affiliating PT and PTA students.\n
\nServes as an advocate for discipline on the multidisciplinary team.\n
\nRepresents department at problem-solving and committee meetings.\n
\n

Minimum Qualifications:

\n

Bachelor's Degree: Physical Therapy (Required)LIC - Licensed in Field - State of Ohio\n

\n

Additional Job Description:

\n

BS in Physical Therapy. Licensed in the state of Ohio. CPR certification that is recognized by the American Heart Association.

\n SPECIALIZED KNOWLEDGE
\n Basic computer skills. 

\n KIND and LENGTH of EXPERIENCE
\n Entry-level new graduate, if facility has at least one veteran Physical Therapist to mentor, otherwise, at least one year of clinical experience.
