Licensed Vocational Nurse (LVN) – Discharge Planner

Job Type: Full-Time
Work Setting: In Person

Position Summary

We are seeking a compassionate and organized Licensed Vocational Nurse (LVN) to join our team as a Discharge Planner in a correctional setting. In this role, you will work closely with the Facility Administrator and Facility Coordinator to develop and implement comprehensive discharge and re-entry plans for individuals preparing to transition back into the community.

As an essential member of the care team, you will help connect patients with medical, behavioral health, substance use treatment, housing, employment, educational, and other community resources that promote successful reintegration and continuity of care.

This position includes an additional 5% weekend differential .

Reporting Relationship

Reports To: Facility Coordinator (Clinical and Administrative Supervision)

Supervises: None

Key Responsibilities

• Coordinate discharge planning and case management services for patients preparing for release.
• Conduct pre-release and post-release assessments to identify medical, behavioral health, and social service needs.
• Develop individualized re-entry care plans that promote successful community reintegration.
• Coordinate community resources, including:
• • Primary and specialty medical care
  • Behavioral health services
  • Substance use treatment
  • Housing assistance
  • Employment resources
  • Educational support
  • Social services
• Ensure continuity of care by coordinating medical benefits, medication prescriptions, and referrals to community providers.
• Collaborate with multidisciplinary teams and external agencies to support patients throughout the transition process.
• Maintain accurate and timely documentation to support patient care and quality improvement initiatives.
• Demonstrate active listening, empathy, and effective communication while working with a diverse patient population.
• Apply critical thinking and problem-solving skills to address barriers to successful re-entry.
• Assist with employee orientation, training, and job shadowing as needed.
• Perform other related duties as assigned.

Qualifications

Required

• Current and unrestricted California Licensed Vocational Nurse (LVN) or Licensed Psychiatric Technician (LPT) license.
• Current CPR Certification.
• Minimum of 2 years of nursing experience or a bachelor's degree in nursing, social work, or a related field, or an equivalent combination of education and experience.

Preferred

• Experience in discharge planning or case management.
• Experience working with justice-involved populations, behavioral health, substance use treatment, or community resource coordination.
• Strong organizational, communication, and documentation skills.
• Ability to work independently while collaborating effectively with interdisciplinary teams.
