Registered Nurse

Looking for RN’s with Home Health, Skilled Nursing and/or Case Management experience with an adult/elderly population. The position is full time with limited travel within the Northern California. Mostly Remote Intial Assessments via video call. Flexible schedule.

Requirements:
1. Must possess a current, unencumbered, active license to practice as a Registered Nurse in the State of California.
2. Reliable Transportation and valid driver’s license and insurance
3. Must be knowledgeable of nursing and medical practices and procedures, as well as law, regulations, and guidelines that pertain to long-term and community based care.
4. Must be able to work both independently and collaboratively with others in a team environment
5. Able to function effectively in a fast-paced environment.
6. Must demonstrate a high level of verbal, writing and listening skills.
7. Computer literacy including use of email, Microsoft Word, Excel
8. Demonstrated commitment to the provision of services for the under served and sensitivity working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic, and cultural origins; disabled, homeless, substance users, HIV (AIDS) infected, and/or physiologically and psychologically impaired.

Essential Responsibilities
· Meet with each client individually and conduct an intake assessments and semiannual reassessments.
· Represents and/or advocates for and with clients in obtaining services.
· Collaborate with the client/patient and care team in the development of an individual service plan and support client/patient in the execution of the plan.
- Attend regular meetings with management and team.
