About the Position

Key Responsibilities

• Provide clinical social work services within an Emergency Department setting
• Conduct psychosocial assessments and develop care plans for patients and families
• Coordinate discharge planning and connect patients with appropriate community resources
• Collaborate with interdisciplinary care teams to support patient outcomes
• Document clinical activity and patient interactions using Epic EMR
• May be required to float within the hospital or to nearby affiliate facilities on a per-unit basis

• Minimum 2 years of clinical social work experience
• Active LCSW or CSW license required (pending licensure not accepted)
• BLS certification required (AHA Traditional - RQI not accepted)
• Experience in an Emergency Department setting required
• Epic EMR experience required
• Must achieve a score of 85% or higher on required competency exams
• Local candidates are welcome; same rates apply

• Valid state license on file at time of submission preferred
• Experience working in a fast-paced acute care or hospital environment
• Familiarity with interdisciplinary team collaboration and complex discharge planning
