Empowering Home Health RN Case Manager

Ohio Living

Ohio Living in Canfield, OH, seeks an experienced RN Case Manager for Home Health to plan, coordinate, and deliver quality care in patients' homes while ensuring compliance with regulatory standards. You will assess patient needs, develop care plans, and oversee their execution with the care team.

The role requires an Ohio RN license, at least one year of acute care or medical-surgical nursing, and proficiency with Windows and MS Office; EMR experience is preferred.
