Geriatrics Nurse Practitioner

The Integrated Home Care Program at Conviva is a creative initiative dedicated to transforming healthcare delivery for high-risk patients in the comfort of their own homes. We aim to reduce avoidable hospitalizations and readmissions by providing comprehensive, patient-centered care to individuals with medical complexity and functional limitations. Our team delivers proactive, coordinated care directly where our patients need it most.

Main Responsibilities:

• You will conduct comprehensive in-home evaluations and examinations
• You will use both in-person and telemedicine capabilities to deliver care
• You will develop and implement personalized treatment plans
• You will manage acute clinical issues in the home
• You will maintain accurate documentation
• You will participate in medical team meetings

You will report to our Chief Medical Officer for San Antonio, Texas. As part of this role, you will work onsite visiting patient's homes in San Antonio, Texas.

If selected for this role, we will require you to undergo a TB screening test.

Required qualifications:

• Nurse Practitioner or Physician Assistant with 1 + years of experience in Home Health, or a related field.

Preferred qualifications:

• Bilingual in Spanish and English
