Our healthcare partner in Grand Junction is seeking a compassionate and skilled Physical Therapist to join their team and make a meaningful impact in the community.

As a Physical Therapist, you'll deliver skilled therapy services directly in patients' homes, ensuring they receive the highest level of care. You'll develop and implement individualized plans of care, perform evaluations, and provide skilled interventions such as gait training and fall prevention. Your role will be crucial in educating patients and caregivers, collaborating with interdisciplinary teams, and maintaining compliance with all regulatory requirements.

• Perform physical therapy visits, including evaluations and discharge visits
• Develop and implement individualized therapy plans based on patient needs
• Provide skilled interventions like gait and balance training
• Educate patients and caregivers on treatment plans and safety strategies
• Communicate patient progress and changes to the care team
• Collaborate with and oversee Physical Therapist Assistants as applicable
• Complete documentation accurately and timely in Homecare Homebase (HCHB)
• Maintain compliance with Medicare, HIPAA, and agency standards
• Need 1 year experience in setting
• Current, unrestricted Colorado Physical Therapist license
• Graduation from an accredited Physical Therapy program
• Valid driver's license and reliable transportation
• Ability to practice independently in a home-based setting
• Proficiency or willingness to learn Homecare Homebase (HCHB)
