Job Description
Title: Physical Therapist

Schedule: 8 hours

Assignment Details:
- Contract Length: 22 weeks
- Guaranteed Hours: 40

Requirements:
- Current State License as a Physical Therapist - Required
- Proof of current CPR certification - Required
- Current driver's license with reliable transportation and insurance - Required
- 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting - Required
- Previous home care experience - Preferred
- Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program - Required

Roles & Responsibilities:
- Conduct clinical assessments to identify patient needs and appropriateness for therapy.
- Document and complete a Plan of Care (POC), progress notes, and other clinical records while revising based on ongoing assessments.
- Plan and provide treatment for neuromuscular, neurological, and orthopedic disorders based on medical referrals and assessments.
- Instruct patients and families on safe transfer techniques and the use of assistive equipment.
- Provide education to patients and families regarding therapeutic procedures adjusted for age, condition, culture, and readiness to learn.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.

Compliance / Notes:
- Must adhere to infection control and universal precautions standards in all patient care activities.
- Accepts clinical assignments that match education and competence.
- Must meet mandatory continuing education requirements.
