Adaptive Home Health is seeking a Registered Nurse (RN) Case Manager for field-based, full-time work across Texas. You will deliver high-acuity home health care, manage OASIS documentation, and coordinate care with physicians, patients and families. Focus on wound care and infusion therapy with autonomy and support from a strong clinical team.
