Contract | Days | 5 x 8-hour shifts | Expected 40 hours/week

Key Responsibilities

• Deliver physical therapy treatment and interventions to residents under the supervision of a licensed Physical Therapist
• Assist in developing and implementing individualized treatment plans in a skilled nursing facility setting
• Document patient progress, treatment notes, and updates in accordance with facility and regulatory standards
• Collaborate with the interdisciplinary care team to support resident rehabilitation goals
• Adhere to a 14-day notice policy as required by the facility

Required Qualifications

• Active Physical Therapist Assistant (PTA) license required
• Skilled Nursing Facility (SNF) experience required
• Ability to start as soon as possible

Preferred Experience / Skills

• Experience working in a travel or contract therapy role
• Familiarity with documentation and compliance standards in a SNF environment
