Job Description
Responsible for carrying out clinical pharmacy services, promoting clinically and evidence-based rational drug therapy, and sound pharmaceutical care through the development of new (or expansion of existing) pharmacy practice programs, drug therapy policies, and other programs. Serves as a clinical pharmacy resource for the health system and its affiliated pharmacy services. Responsible to serve as a preceptor for the residency program, and for students from local colleges of pharmacy. Conducts research and other scholarly activities throughout the year with the goal of publishing. Supervises and guides pharmacists on the optimal provision of care to ensure professional development, and through use of their independent judgment, directs and disciplines their employees. Other responsibilities include patient care rounds, clinical interventions, medication use evaluations and institutional committee membership.
Qualifications
Requirements:
Doctor of Pharmacy Degree required
Current NYS Pharmacist license and registration
ASHP-Accredited PGY-1 required, PGY-2 residency preferred
Board certification (BCPS, BCCCP) strongly encouraged
Hospital pharmacy and/or clinical experience; management experience preferred

The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $127,044.00 - $190,565.00 Annually. Actual salaries depend on a variety of factors, including experience, education, and operational need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
Responsibilities
Roles & Responsibilities:
Participate in daily multidisciplinary internal medicine hospitalist rounds
Provide accurate and timely medication information to healthcare staff and patients
Provide clinical and operational pharmacy support to pharmacy and medicine teams
Collaborate with the Director and Pharmacy and Associate Director of Pharmacy to develop and implement clinical pharmacy programs and services in support of the pharmacy department mission
Serve as an active participant in local and system committees in related clinical practice areas as assigned
Actively participate in the Antimicrobial Stewardship program efforts
Contribute to the Transitions of Care services and programs
Identify, design, and analyze drug utilization reviews to promote rational drug therapy and determine action plan based on results
Participate in departmental and hospital quality and cost-savings initiatives
Sustain the hospital formulary, minimizing non-formulary procurement, utilizing therapeutic interchanges and promote rational drug therapy selection
Work collaboratively with other health care providers and the clinical team members to develop and maintain a sound formulary system to support evidence based pharmaceutical care and cost-effective drug utilization.
Coordinate formulary reviews, drug use evaluations, and quality assurance programs.
Serve as a preceptor for pharmacy students and PGY1 pharmacy residents
Develop and implement systems and procedures to maintain an efficient and effective workflow within the pharmacy and/or assigned areas
Provide direction and guidance for the safe processing and dispensing of all medications for assigned area
Address issues or services requiring intradepartmental coordination proactively
Ensure an adequate inventory of pharmacy products and supplies to meet department demands
Ensure the department is in compliance with all federal, state and local regulations, accrediting agencies; and Mount Sinai Heath System policies and procedures.
Contribute to managing various personnel functions such as hiring, training, performance appraisals, coaching, disciplinary actions, and terminations.
Exchange information and provide input for inter-and-intra departmental meetings
Serve on a rotating basis, as an on-call administrator and troubleshoot issues that arise evenings, weekends, and holidays.
Maintain adequate documentation to satisfy JCAHO requirements or other regulatory agencies
Perform other duties as assigned
