We are hiring an Occupational Therapist (OT) for a Home Health position providing skilled care to patients in their homes in the Castle Shanon area. This position is eligible for $16,000 total bonus potential, including a $10,000 first year success bonus to reward the accomplishment of key achievements during your first year as well as an additional $6,000 sign-on bonus paid within the first 18 months of employment! To receive these bonuses, you must be in good standing & meet performance expectations, and in the role and status for which you were hired!

What You’ll Do:

\n

\n
• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
\n
• Perform comprehensive patient evaluations focused on activities of daily living (ADLs), upper extremity function, and home safety, developing individualized treatment plans to promote independence and quality of life
\n
• Educate and empower patients and caregivers on adaptive techniques, use of assistive equipment, energy conservation, and home safety strategies to support safe, confident participation in daily routines
\n
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
\n

• Work independently, but never alone

• Travel to patient homes within assigned territory

OT Qualifications

\n

\n
• Active Occupational Therapist, OT license in the state you work
\n
• One year of experience as an Occupational Therapist and the clinical expertise to deliver quality patient care
\n
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
\n

\n
