Hospice Registered Nurse (RN) Case Manager

MetroWest (Southborogh, Framingham, Northborough, Marlborough and surrounding towns)

Ideally, we are looking for two RN Case Managers: 1 for the Northshore and 1 for the Southshore. Both covering Boston and surrounding towns.

We are seeking a compassionate and detail oriented Registered Nurse (RN) Case Manager to join our experienced hospice team serving patients and families throughout the greater Boston and surrounding areas.

As part of a highly supportive group of seasoned hospice clinicians, you will have the autonomy to manage your own caseload while enjoying the camaraderie, respect and collaboration of a team that truly works together.

What you'll do:

• Provide comprehensive, compassionate care to patients and families in their homes or facilities.

• Coordinate interdisciplinary care and ensure timely communication among team members, patients and those who care for them.

• Advocate for patients' wishes while maintaining the highest standards of clinical excellence. 

• Manage visit schedule, Medicare and organizational compliant documentation with attention to detail.

What We're Looking For:

• Registered Nurse - RN - Current MA RN licensure and valid driver's license

• At least 1 year Hospice an/or Palliative care experience preferred

• Strong communication, self-organization and documentation skills

• A compassionate presence, positive energy and a team spirit

• CHPN certification preferred (or willingness to pursue...We will help)!
