Physical Therapist - Travel Contract

Physical Therapist — Crawfordsville, IN (Hospital)

Provide skilled physical therapy services to outpatient and occasional inpatient populations in a hospital-based outpatient department. Manage orthopedic and post-surgical conditions through evaluation, individualized treatment planning, and progressive therapeutic interventions. Collaborate with interdisciplinary team members to optimize patient function, document patient progress in the electronic medical record, and follow facility policies and procedures. Candidates will be expected to complete any assigned HR modules prior to the first day and report to the outpatient entrance as instructed.

Minimum Qualifications

• Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Indiana state physical therapy license required to start the assignment.
• BLS certification required from AHA or ARC prior to patient contact.
• Minimum 1 year of outpatient physical therapy experience required, including orthopedics.
• Experience with modalities and equipment such as ultrasound, electrical stimulation, laser, and Hivamat preferred.
• Proficiency with Epic or similar electronic medical record systems preferred.
• Must be a team player with a strong work ethic.

Preferred Qualifications

• Additional training or demonstrated skill in manual therapy techniques preferred.
• Experience treating a caseload with approximately 70% orthopedic diagnoses preferred.

Assignment Details

• Shift hours: Monday–Friday, 08:30–17:00.
• Weekly guaranteed hours: 36 hours per week.
• Primary setting: Outpatient clinic with approximately 5–10% inpatient coverage.
• Patient ratio: typically 1:1.
• Equipment available: Hivamat, ultrasound, electrical stimulation, Bits, laser.
• Float requirements: none.

Provide evidenced-based physical therapy interventions that support functional recovery and return to activities of daily living. Maintain accurate, timely documentation in Epic and participate in interdisciplinary rounds as needed. Support quality initiatives and patient satisfaction goals through strong clinical practice and patient education.
